Administrative Assistant – Custodial Census Specialist

About the role

The Custodial Census Specialist manages the daily resident census for assigned skilled nursing facilities, ensuring that all information is accurate, up‑to‑date, and compliant with payer requirements. This role acts as a liaison between facilities, managed‑care organizations, and internal teams to resolve eligibility and authorization issues while supporting reimbursement processes.

Key responsibilities

• Maintain the daily custodial care census, updating admissions, discharges, transfers, and payer changes.
• Verify insurance eligibility, benefits, and appropriate payer assignment.
• Review and track authorization requirements for custodial residents.
• Collaborate with facilities, insurers, and internal departments to resolve census discrepancies.
• Respond promptly to payer notifications and ensure resident records remain accurate.
• Upload and manage discharge documentation in internal systems.
• Identify and escalate potential reimbursement risks or documentation deficiencies.
• Prioritize work queues to meet departmental turnaround times.
• Ensure compliance with payer guidelines, HIPAA, and company policies.
• Participate in process‑improvement initiatives and special projects.

Required profile

• Working knowledge of insurance plans (preferred).
• Experience with skilled nursing facilities, long‑term care, or post‑acute care (preferred).
• Proficiency with Microsoft Office Suite, especially Excel and Outlook.
• Familiarity with electronic medical records (EMR), payer portals, and healthcare software systems (preferred).
