Key responsibilities

• Manage and follow up on active healthcare cases, tracking patient progress.
• Provide proactive leadership to keep team workflows organized and efficient.
• Serve as the bilingual point of contact for patients and families, delivering updates and compassionate support.

Required profile

• At least 2 years recent experience in healthcare case management or clinical coordination.
• Strong capabilities in care coordination workflows, case tracking software, and team leadership.
• Advanced English speaking and writing skills; bilingual English/Spanish required.
• Self‑starter attitude with a desire to grow and innovate.
