Care Management Scheduler

About the role

The Care Management Scheduler coordinates patient appointments, follow‑up visits, referrals and diagnostic services across primary and specialty care. Working closely with providers, clinical staff, patients and insurers, the Scheduler ensures timely access to care and helps maintain continuity for patients throughout their healthcare journey.

Key responsibilities

• Schedule, reschedule and confirm patient appointments for primary care, specialty care, labs, imaging and referrals.
• Contact patients by phone or electronic means regarding upcoming, missed or overdue appointments.
• Assist patients in arranging follow‑up care after office visits, hospitalizations, ER visits or discharge from inpatient, rehab or skilled‑nursing facilities.
• Identify overdue services (labs, diagnostics, referrals) and coordinate appropriate follow‑up.
• Communicate appointment changes, cancellations and preparation instructions clearly to patients and the care team.
• Coordinate referrals and liaise with specialty offices, diagnostic centers and laboratories.
• Verify insurance information and work with payers to support appointment and referral coordination.
• Document all scheduling activity, patient communication and referrals in the electronic medical record.
• Monitor schedules for no‑shows and openings, helping to fill available slots.
• Participate in team meetings, huddles and quality‑improvement initiatives.

Required profile

• Strong organizational and time‑management abilities.
• Excellent verbal and written communication skills for interacting with patients, providers and insurers.
• Attention to detail and ability to maintain accurate records in an electronic medical‑record system.
• Commitment to patient confidentiality and compliance with HIPAA regulations.
