Team Leader / Case Manager – Healthcare (Onsite in Santiago)

About the role

We are looking for a bilingual healthcare professional to lead our on‑site team in Santiago de los Caballeros. The Team Leader/Case Manager will coordinate patient cases, support families, and ensure the care team stays organized and focused on delivering high‑quality outcomes.

Key responsibilities

• Monitor, follow up and manage active healthcare cases to guarantee continuous patient care.
• Lead the care team, keeping daily objectives on track and maintaining a high level of organization.
• Communicate clearly and empathetically with patients and families, providing updates, education and support.
• Identify operational bottlenecks, streamline patient intake processes and keep accurate records.

Required profile

• Minimum 2 years recent experience in healthcare case management.
• Proficiency with case‑tracking systems and organizational tools.
• Advanced English and Spanish speaking and writing skills (bilingual required).
• Current residence in Santiago de los Caballeros with ability to work 100 % onsite.
