Remote Population Health Navigator and Scheduler

About the role

We are looking for a Remote Population Health Navigator and Scheduler to join our care management team. In this role you will support patient engagement, coordinate appointments, and help drive population‑health initiatives for individuals with chronic and complex conditions.

Key responsibilities

• Contact patients to encourage participation in their care plans.
• Schedule and coordinate appointments, follow‑ups and home‑care services.
• Assist Care Managers with outreach, daily readings, assessments and compliance monitoring.
• Track patient activity, prepare weekly reports and maintain up‑to‑date scheduling records.
• Escalate missed appointments or concerns to the appropriate clinical team.
• Communicate with patients, caregivers, providers and internal staff as needed.
• Provide administrative support for care coordination and population‑health projects.

Required profile

• High school diploma (Bachelor’s degree preferred).
• 2–3 years of experience in healthcare, care coordination, patient navigation or scheduling.
• Excellent verbal and written communication skills.
• Strong customer‑service orientation and ability to handle confidential information.
• Detail‑oriented, organized and able to manage multiple priorities in a fast‑paced environment.
